HOW TO.

Key Points

  1. Every resident will need their own unique survey.

  2. IMPORTANT: this survey will be accessed through respondents' phones so be specific and concise with your questions.

Survey Monkey® 

 

Getting Started

This system uses two operating systems: Survey Monkey® and Microsoft Tag® (or a QR reader platform).

 

Both systems are free, but upgrades are available as needed for a nominal fee. 

1. Create an account:

 

Survey Monkey offers free basic accounts as well as upgraded/professional accounts with small annual fees.

 

 

2. Create a template survey:

 

Once you have logged in select: “+ Create Survey” under “My Surveys”

 

From here, you can choose to create a new survey if you are setting up your template survey, or you can choose to copy an existing survey once a template survey has been made.

 

Create your template survey by adding questions. Survey monkey allows multiple question types including multiple choice, drop­‐down menus, free text responses, etc.

 

Once your survey is complete, preview it to verify it.

Once your template is created, you are ready to get started.

 

REMEMBER: Keep your questions specific and concise.  This survey will be accessed via smartphones.  See example.

 

 

3. Create a resident/fellow/faculty survey:

 

Once your template surveys have been created, individual surveys are easily generated by copying a template survey:

  • Return to “My Surveys”-­‐ Select “+ Create Survey” as above.

  • Select “Copy an existing survey” and choose the survey you wish to copy

  • Change the nickname of the survey (for example: Jane Doe Resident Evaluation)-­‐ Choose "Continue"

  • Select the Collect Responses Tab

  • Choose "Web link", then "Next Step"

  • Customize the name of your survey. Choose an easy way to keep track of surveys (i.e. resident's name or common ID number) This link will be used when creating an individual's tag in the Microsoft Tag portion.

  • Click on "Save"-­‐ On the left hand column click on "Change Settings"

  • Under the question: "Allow Multiple Responses?" Select Yes -­‐Click on "Save Settings"

       This final step is important for ensuring the survey functions properly.

 

  • This survey is now ready for use. Remember each person to be evaluated will need their own unique survey. 

 

 

QR Reader

(Microsoft Tag®)

1. Create an account:

 

Sign In (Right Upper Corner) and create a Windows Live ID with a working email address. Follow the instructions to set up an account.

 

2. Create a new tag:

 

  • Go to MY TAGS: Tag Manager (This shows tags that have already been created. These tags can be edited, reports viewed, etc. from this location.)

  • Click on the Blue Box “Create a Tag”

  • Fill out this page (below) with the following:

         -­‐  Tag Title: name the tag (i.e. Jane Doe Evaluation)

         -­‐  Tag Type: URL

         -­‐  End Date: No end date

         -­‐  Mobile URL: Use the URL under “Weblink” that was created in Survey Monkey. This is the custom link that will be unique to each individual evaluation.

         -­‐  Click Save

  • Make a QR code

         -­‐  Click on the blue arrow under Download

         -­‐  Choose from Tag barcode, Custom barcode, or QR barcode (I have used Custom barcode with “render tag in black and white” for our tags.)

         -­‐  File Format: PDF

         -­‐  Tag Size: 1.25

         -­‐  Click: Download

  • A new window with a new code: Copy this pdf and crop just the QR code onto a previously created ID template. 

ID Template

  • Create an ID template

  • The unique QR code will be cropped and added to the ID template

  • Print your ID's and begin using your own Resident Evaluation X-press System
